Your First Visit

From beginning to end, Michigan Oral Surgeons wants to make your experience with us as smooth as possible. We encourage you to review the information below prior to your first appointment.

Make the Most of Your First Appointment

Depending on your condition, your initial appointment may consist of a consultation and examination or immediate treatment. Your oral surgeon will go through the exam process, explain your diagnosis, and review your treatment options. This is a great time to ask any questions you may have.

Please arrive a few minutes early to check in and fill out any paperwork. To assist in the registration process, new patients can complete and submit their Patient Intake forms by registering online.

Please Bring These Items With You

At the time of your appointment, you may assist us by providing the following information:

  • A treatment request from your dentist or referring doctor, if available
  • A list of current medications, dosage, and reason for use
  • All known allergies
  • All dental and medical insurance information
  • Photo ID for all patients over 18 years old (or parent/gaurdian for minors)
  • Mask

Note to Parents of Minors

All patients who are minors (under the age of 18) must be accompanied by a parent or legal guardian. If this may be a problem, please contact our office for possible arrangements.

What to Expect

If your dentist has taken current X-rays, you may request that they are forwarded to our office. You may also bring them with you to the appointment. If additional films are necessary, they will be taken at our office during your appointment.

If you are receiving immediate treatment involving IV sedation for surgery, an adult must accompany you to the surgical appointment. This person must remain at our office for the duration of the surgical procedure and drive the patient home. Patients are required to abstain from eating or drinking anything for 6 hours before their scheduled procedure.

Our team wants to make your surgical experience as easy and pleasant as possible. Michigan Oral Surgeons accepts several methods of payment, including:

It is the patient’s responsibility to provide our office with complete and accurate insurance information. Covered procedures vary with each insurance company, and our staff will assist you in determining eligibility and estimated benefits. Payment for deductibles, co-pays, and non-covered benefits are due at the time of service.

Please note all patients are fully responsible for all charges regardless of insurance coverage.

For your convenience, we will gladly submit an insurance claim for rendered services. We will ask you to sign a financial policy reflecting acknowledgment and understanding of our policies at your initial visit.

If you have specific insurance questions, we are happy to answer them. Please call the office where you will be receiving your treatment to speak with one of our front office staff members.
